PHP Focus

At Echoes of Hope LLC, treatment planning is a collaborative and dynamic process tailored to the unique needs of each client enrolled in our Partial Hospitalization Program (PHP), outpatient, or housing services. Treatment plans are strengths-based, person-centered, and culturally responsive, designed to support recovery from substance use, co-occurring disorders, and the challenges of homelessness. 
 

Treatment Planning

Initial Treatment Plan Requirements

 A preliminary treatment plan is initiated within 24 hours of PHP admission 
- A comprehensive treatment plan is completed within 14 calendar days of admission - Plans are developed with input from: 
-The client 
- Assigned clinician or case manager 
- Peer support (if applicable) 
- Any outside providers with signed ROI

Person Centered Approach

Treatment plans reflect: 
-The client's voice, preferences, and cultural needs 
- Recovery goals such as housing, employment, or sobriety 
- Consideration of trauma history and coping strategies 
- Integration of support systems, including family or sponsors when appropriate

Treatment Plan Components

Each plan must include the following: 
- Presenting problems (behavioral health, housing, substance use, etc.) 
- ASAM level of care justification 
- Diagnoses (per DSM-5-TR) 
- Client-identified goals 
- Measurable objectives with timelines 
- Specific interventions assigned to staff 
- Discharge planning considerations

Review and Updates

 Plans are reviewed every 14 days in PHP (or more frequently if needed) 
- Updates are made based on: 

- Clinical progress 
- New symptoms or barriers 
- Legal, housing, or medication changes 
- Updates must include client input and updated signatures

Discharge and Transition Planning

Begins at admission and is revisited regularly 
- Plans address aftercare needs: 
- Outpatient therapy 
- Medication management 
- Housing or shelter options 
- Community support (e.g., NA/AA, job programs) 
- A written discharge plan is included in the final treatment review
