PHP; Partial Hospitalization Program
The Partial Hospitalization Program (PHP) at Echoes of Hope LLC offers intensive, structured treatment for individuals with moderate to severe substance use disorders and/or co-occurring mental health conditions. PHP is designed to serve clients who require a high level of clinical support but do not meet inpatient criteria. The program serves as a vital transition point from detox, residential care, incarceration, or unstable housing environments.
PHP is trauma-informed, recovery-oriented, and emphasizes stabilization, coping skills, housing support, and reintegration into community life.
PHP Program Overview
Eligibility and Admission Criteria
Policy Statement:
Clients are admitted to PHP based on medical necessity and a clinical assessment using ASAM criteria or equivalent. Admission decisions are made by a licensed clinician.
Eligibility Includes:
​ 18 years of age or older
- Diagnosed with a substance use disorder (with or without co-occurring mental illness)
- Able to participate in a 5-day-a-week program
- Stable enough to attend programming without inpatient monitoring
- Motivated to engage in intensive services and case management

Treatment and Clinical Oversight
Treatment plans are developed and reviewed every 14 days or as clinically indicated.
Clinical Staff Include:
- Licensed Therapist(s)
- Clinical Director
- MAT Nurse or Prescriber (if applicable)
- Peer Recovery Coach
- Case Manager

Attendance and Discharge Planning
Policy Statement:
Attendance and engagement are essential in PHP. Clients must participate in scheduled programming unless medically excused.
Procedure:
- Clients are expected to attend at least 5 days/week.
- Absences must be reported and documented.
- Repeated no-shows will trigger a clinical review and discharge planning meeting.
- Discharge planning begins at admission and includes aftercare coordination with outpatient services or housing supports.
Program Structure
Policy Statement:
PHP services are provided Monday through Friday, minimum of 5 hours per day, totaling 25+ hours per week.
Daily Services May Include:
- Group therapy (CBT, DBT, relapse prevention, trauma recovery)
- Individual therapy
- Psychiatric evaluation and medication management (if needed)
- Case management
- Life skills and recovery education
- Peer support services
- Wellness and vocational activities
- Medication-Assisted Treatment (MAT) coordination

Program Schedule
Sample Schedule (Monday-Friday):
- 9:00 AM - Morning Check-In & Goal Setting
- 9:30 AM - Group Therapy
- 11:00 AM - Psychoeducation / Life Skills
- 12:00 PM - Lunch (on-site or provided by client) - 12:30 PM - Peer Support or Individual Counseling - 2:00 PM - Wrap-Up, Journaling, Recovery Planning

Documents and Compliance
Daily progress notes completed within 24 hours
- Weekly team meetings to review client progress
- Random drug testing (min. 1x/week)
- Compliance with Medicaid and OFLAC PHP standards
