Behavioral Health Outpatient Program

PHP Aligned

Echoes of Hope LLC delivers integrated outpatient behavioral health services as a core component of its Partial Hospitalization Program (PHP). These services are designed to address the mental, emotional, and behavioral health needs of individuals with substance use disorders, often compounded by homelessness, trauma, and system involvement. 
Outpatient services within the PHP structure are delivered on-site, embedded in the daily schedule, and tailored to support stabilization, treatment engagement, and long-term recovery.

Eligibility Criteria

Policy Statement:

​Outpatient services are provided as part of the PHP model and are available to all PHP- enrolled clients based on clinical need and treatment planning. 

Eligibility Includes: 
- Enrollment in Echoes of Hope's PHP 
- Diagnosis of substance use disorder or co-occurring behavioral health condition 
- Willingness to participate in therapeutic services as outlined in the treatment plan 
- Clinical appropriateness for outpatient-level interventions (e.g., group therapy, individual sessions) 

Medical Form

Treatment Planning and Coordination 

Outpatient services are planned and delivered under the umbrella of the client's PHP treatment plan 
- Plans are reviewed every 14 days by the treatment team 
- Clinical updates are shared during weekly multidisciplinary staffing meetings

 Documentation and Compliance 

 All outpatient services must be documented in the client's medical record within 24 hours 
- Records must include service type, duration, participant engagement, and clinical progress 
- Notes are audited weekly for compliance with OFLAC, Medicaid, and internal standards 
- All documentation must link back to PHP enrollment and the individual treatment plan
 

Scope of Services (Within PHP)

Behavioral health outpatient services delivered through the PHP may include: 
- Individual Therapy (CBT, trauma-focused, motivational interviewing) 
- Group Therapy (relapse prevention, coping skills, grief & loss) 
- Family Counseling (when clinically indicated) 
- Psychoeducational Classes (recovery topics, emotional regulation) 
- Peer Recovery Support Sessions 
- Crisis Counseling and Safety Planning 
- Case Management Coordination 
- Discharge and Aftercare Support 

These services are fully integrated into the PHP daily schedule. 

Therapy

Frequency and Clinical Requirements

 Clients will participate in 20+ hours/week of combined behavioral health, peer support, and recovery activities 
- Individual therapy: at least 1x/week 
- Group therapy: at least 3-5x/week 
- Clinical staff must document all services daily and update treatment progress weekly
